Scranton, PA. A new peer-reviewed research study published in the Journal of Medical Internet Research (JMIR) found that The GUIDE App, an anonymous mobile wellness tool created for first responders, veterans, and frontline workers, measurably improves mental health and emotional wellbeing.

In a randomized controlled trial, regular use of The GUIDE App reduced symptoms of depression by 32 percent and anxiety by 9 percent over a four-week period.

The study followed 115 participants, including law enforcement, EMS personnel, firefighters, veterans and other frontline responders. Forty-two percent reported a military affiliation. At the start of the trial, more than 70 percent showed symptoms consistent with moderate to severe anxiety, and 27 percent with moderate to severe depression, which are rates significantly above those seen in the general U.S. population.

The trial represents the first rigorous evaluation of a digital mental health tool specifically designed for these high-risk communities.

The GUIDE App combines complete anonymity, secure technology, short evidence-based lessons, moderated peer groups, and simple wellness tracking tools. With just 15 minutes a day, participants engaged in skills and conversations shown to reduce stress and improve coping.

We built GUIDE so that no firefighter, police officer, medic, or veteran has to struggle alone. — Patrick Sandone, Founder of The GUIDE App

Developed with direct input from first responders and veterans, GUIDE was built to address the barriers these professions face when seeking support, including stigma, confidentiality concerns, shift schedules, and the cultural pressure to push through difficult experiences.

Additional Study Insights

• Engagement was high, with 93 percent of participants completing the full trial
• Male and military-affiliated participants demonstrated particularly strong improvement
• The app was rated highly for trust, usability, and relevance to participants lives

Participants also shared positive feedback about their experience using the app. One noted, “I looked forward to documenting my feelings daily and receiving support from others who understand my line of work.” Another said, “The topics really relate to us in law enforcement. I felt engaged, and I appreciated the anonymous small forum where we could talk openly.”

A previous pilot study of The GUIDE App showed similar outcomes, including increased positive emotions, improved motivation, and reductions in negative emotions and loneliness. That study also documented a 29 percent average decrease in anxiety among regular app users.

Today, The GUIDE App supports tens of thousands of first responders, veterans, and frontline workers across the United States.
